Call the Christian Family Medicine location nearest you. Our front desk staff can verify your insurance over the phone and help with questions about our services.
Check Your Insurance Card
Look at the back of your insurance card for the member services number. Call them directly and ask if Christian Family Medicine & Pediatrics is an in-network provider in your area.
Bring It to Your Visit
Bring your insurance card to your appointment. Our team will verify your coverage at check-in and let you know about any copays or out-of-pocket costs before your visit begins.
Self-Pay & Uninsured Patients
We believe cost should not be a barrier to quality healthcare. If you don't have insurance, our self-pay program offers transparent, affordable pricing for all of our services.
As federally designated Rural Health Clinics, we offer a sliding fee scale based on family size and income for qualifying patients.
